Your NHS Data Matters – National Data Opt-Out

πŸ›‘ Opting Out of Data Sharing – Your Choices

At Malthouse Surgery, we respect your privacy and want you to be informed about how your health data is used. You have the right to opt out of your confidential information being used for planning or research. There are two types of data opt-outs available:

πŸ”Ή Type 1 Opt-Out – Data from your GP record

This stops your GP-held information from being shared with NHS Digital and other organisations for purposes beyond your individual care (such as planning or research).

βœ… How to opt out:

  • Complete a Type 1 Opt-Out form and return it to the surgery.

  • We will add a code to your record: 9Nu0 – Dissent from secondary use of GP patient identifiable data

You can request a copy of the form at reception or download it here.

πŸ”΅ National Data Opt-Out – Data from hospitals and other NHS services

This controls whether your confidential patient information from across the NHS (not your GP record) can be used for:

  • NHS planning and commissioning

  • Research into disease trends and outcomes

This is managed centrally by NHS England, not by your GP practice.

βœ… How to opt out:

ℹ️ Important Notes

  • These opt-outs do not affect your individual care or treatment.

  • You can change your choice at any time.

  • If you’re under 13, a parent or guardian must make the opt-out on your behalf.
