Do I need a fit note?
- Off work 7 days or less: you do not need a fit note. Use your employer’s self-certification process.
- Off work more than 7 days (including weekends/bank holidays): you do need a fit note.
A fit note (sometimes called a sick note) can also support a phased return to work or reasonable adjustments, and may be used for Statutory Sick Pay (SSP) or benefits claims.
Who can issue a fit note?
Fit notes can be issued by a GP or by certain other qualified healthcare professionals in primary care (such as nurses, pharmacists, and physiotherapists). If you were treated in hospital, your hospital team should provide your initial note.
How to request a fit note

What to include in your request
- Your full name, date of birth, and best contact number.
- The health problem affecting your work.
- When the problem started and how it affects your job.
- Dates you need the fit note to cover (start and end date, or “from” date if an extension).
- Whether this is a new fit note or an extension.
- Any workplace adjustments that may help (e.g., lighter duties, reduced hours).
Processing times & delivery
- We usually process fit note requests within 2 working days. Complex cases may take longer.
- Fit notes are normally issued electronically. We can provide a printed copy if needed.
Extensions, backdating & private notes
- Extensions: request before your current note expires where possible.
- Backdating: may be possible based on clinical evidence—please give full details in your request.
- Private notes: For school/college, gym, or insurer letters not covered by NHS services, an administrative fee may apply. Ask reception for current charges.
Common questions
My employer is asking for a note sooner than 7 days—can you provide one?
The NHS fit note is only required after 7 days. For shorter absences, use employer self-certification. If they still require a letter, this may be a private (non-NHS) request and a fee may apply.
I was in hospital—who gives my fit note?
The hospital team should issue your initial fit note to cover your expected recovery time. Contact them if it wasn’t provided; we can help with extensions once you’re under our care.
Can a fit note say “may be fit for work” with adjustments?
Yes. We can advise adjustments such as amended duties, altered hours, or a phased return where clinically appropriate.
