Long Term Conditions
As your GP surgery we play a significant role in managing your long-term condition. We believe in working with you and specialists in the community to provide holistic, personal, and high-quality care.
Long term conditions or chronic diseases are conditions for which there is currently no cure, and that are managed with drugs and other treatments.
All patients with a long-term condition should have an annual review. The first part of this is with a health care assistant or a nurse. At this appointment we will take a blood sample, check your blood pressure, and update other health data.
A GP will review your results, looking at your medical history and medications, and consider how together we could improve your health/management of your long term conditions. This may include health advice, medication changes or referrals to a community service.
Your second contact with us will be to share and discuss the GP’s recommendations with you. This appointment will also be with a healthcare assistant or a nurse. If you have more than one long term condition, we aim to make this second appointment with a specialist nurse or GP.

Cervical Screening: Smear Tests
Women aged between 24 and 64 should have a cervical screening every 3 to 5 years to help prevent cervical cancer. The screening is quick and painless and can be done here in the practice.
If you are aged over 24 and have never had a smear test, or if it has been more than 3 to 5 years since your last screening, you should arrange an appointment with our Practice Nurse. You should not have the test while you are having a period or in the 4 days before or after your period as this can affect the sample.
Social Prescribing
Social prescribing is an opportunity to talk through any worries that might be affecting your health and wellbeing in a safe environment. The service supports you to make positive changes in your life by linking you up with groups, activities and public services that can help.
The support we refer you to could include; finding volunteering opportunities, improving emotional wellbeing including befriending schemes, peer support or arts and leisure activities, social aspects of healthy living, which includes non-medical support to manage a long term, health condition, and practical support at home to keep safe.
If you feel you need this support, please contact us and ask us for a referral to be made to the social prescribers for you.
Learning Disability Support
As a surgery we offer support to those with Learning Disabilities, We have step by step east read guide for nurse’s appointments. These are located in our waiting room and below.
We have welcome packs available to give to patients on request. Please ask Reception.
We have visual cards for non verbal patients if they would like to book anything.
We offer text message notifications for all learning disability reviews the day before the appointment. At request we can offer texts for all appointments booked here.
If you have a learning disability you are invited in for a yearly health check at the surgery. This is with our Health Care Assistants who talk about your health and do some simple tests. This is to ensure you are feeling well and help you to stay healthy.
