Carer’s Champions in Surgery
We have recently developed a team of champions to help with carers at our surgery. The lovely ladies Jasmyn and Jacinta are in the surgery throughout the week for any questions or queries. They have created a carer’s pack with useful information regarding Forms, Local Services, Respite Care, Power of Attorney, and consent to disclose medical records. This packs are located in the surgery. Please speak to Reception or the Champions to pick one up.
